Choose this if you're ready to begin care. Your first visit includes a full history, a thorough assessment, and time to start treatment. After you book, you'll receive a secure intake form to complete beforehand, so I can review your history before we meet.
Initial evaluation — $175 founding-patient rate (regularly $200) · 120 minutes

- Payment Payment is due at time of service; cash or check preferred, cards accepted. Receipts are available and typically eligible for HSA/FSA reimbursement.
Cancellation & No-Show Policy
Because each visit is a dedicated one-on-one appointment, I ask for at least 24 hours' notice to cancel or reschedule. Late cancellations (less than 24 hours) and missed appointments may be subject to a $50 fee. Life happens, though — if something comes up, just reach out, and I'll always do my best to work with you.
A note on urgent or emergency concerns
Conway Physical Therapy isn't set up for emergencies. If you're experiencing a medical emergency, call 911. For a new, unexplained, or rapidly worsening symptom, please start with your physician or an appropriate medical provider — and you're always welcome to reach out afterward if physical therapy is the right next step.
